The existing accesses to thyroid gland do not always fully meet the requirements of a low injury rate and a low invasiveness. The aim of the study is to evaluate the results of surgical treatment of benign thyroid gland diseases using the developed upper cervical approach (patent of the Russian Federation for an invention № 2726006 est. 08.07.2020).

We used randomized prospective controlled trial. The research is based on the evaluation of treatment results of 52 patients with the benign thyroid diseases, which were divided in two groups. The first group consisted of patients who underwent upper neck access to the thyroid gland and the second group – with Kocher access to the gland. The study has revealed: the developed upper cervical approach reduces the area of the wound surface by 26.6±11.1 %, allows the reliably visualizing of both recurrent and superior laryngeal nerves, reduces the frequency of postoperative complications by 4 times, increases the cosmetic component of the operation.
