The study was aimed at evaluating the structure of nosocomial pathogens and the efficacy of the local Antimicrobial Stewardship Program (ASP) in the intensive care units (specialized hospital). The study involved a retrospective and prospective analysis on 120 microbiological swabs and cultures of biological fluids (blood, sputum, urine) obtained from patients in specialized care departments and intensive care units. Identification of microorganisms and determination of their antibiotic susceptibility were performed employing the disk diffusion method along with automated systems. The study revealed that Gram-negative bacteria predominated in the pathogen structure, demonstrating a high level of resistance to Generation 3 and 4 cephalosporins and fluoroquinolones. An increase in the share of carbapenemase-producing strains was also identified. The ASP protocol, which includes mandatory local monitoring and restriction of empirical prescription of reserve antibiotics, led to an 18.5% reduction in the selection frequency of multidrug-resistant strains and optimized pharmacotherapy costs.
