Diagnosis and treatment of acute poisoning with 1.4-butanediol complicated by delirium

[Psychiatry]
Andrey Georgievich Sinenchenko; Alexey Lodyagin; Bair Vasilievich Batotsyrenov;

Examination of 40 male patients aged 20 to 45 (32.7±1.1) years with 1.4-butanediol poisoning complicated by delirium revealed the predominance of mixed changes in the acid-base state in the form of respiratory acidosis and metabolic lactic acidosis. The use of Cytoflavin solution (40 ml per day) was accompanied by a significant change in the acid-base state, a decrease in lactate in capillary blood by the 3rd day of therapy from 3.9 to 2.8 mmol/l, a decrease in the severity of delirious symptoms by the 5th day from 21.2 to 17.3 points, on the DRS-R-98 scale, an increase in the oxygen consumption and the release of carbon dioxide by the 7th day by 21.08 and 7.3 %, in comparison with the control group. Thus, Cytoflavin, containing succinic acid in its composition in the form of Na, N-methylglucammonium succinate, is able to compensate metabolic lactate acidosis at an earlier date, stabilize the acid-base state by increasing oxygen consumption and activating of aerobic oxidation processes.
