The frequency of cystic transformation of the bile ducts is 1:100 000–150000 people and in 97 % of cases there are type I cysts on Todani. Type II cysts account for less than 2 %. The article presents a clinical observation of a 5-month-old patient with a diverticulum of the common hepatic duct. The diverticulum was located at the confluence of the right and left hepatic ducts. The clinical manifestations of such a cyst in young children are not specific and often minimally expressed or absent. The main method of diagnosis and treatment is surgery. The complete resection of the cyst with the formation of portoenteroanastomosis on a prepared Roux loop allowed to achieve a high «quality of life» of the child after surgery, minimizing possible postoperative complications and potential carcinogenesis.
