The method of pilonidal sinus treatment in patients with high butocks configuration

The treatment result have been studied of 80 patients with pilonidal sinus (PS) complicated by secondary fistulas of the gluteal-sacrococcygeal region located on the inner semicircle of the buttocks was carried out. In 37 (46.3 %) patients the pilonidal sinus was excised, all fistulas were dissected and excised with separate skin incisions, leaving the wounds open. The operation for excision of fistulas with pathologically altered tissues, followed by closure of the wound with skin grafting with a perforated flap was performed in 3 (3.7 %) patients. In 40 (50.0 %) patients (the main group), a modification of the operation worked out by the authors. The proposed method of PS treatment made it possible to reduce the number of complications by 2 times, relapses by 4 times, and reduce the duration of inpatient treatment by 2 days.
