Portal vein thrombosis in the practice of the internist

Portal vein thrombosis (PVT) is a type of venous thrombosis that causes impairment or termination of blood flow inside the portal vein. PVT is caused by a combination of local and systemic risk factors. PVT in cirrhosis is accompanied by an increased risk of both hemorrhagic and thrombotic complications. PVT can be classified according to the time of development, localization, pathophysiology and complications. Ultrasound, CT and MRI play an important role in the evaluation of this condition. The range of treatment for PVT includes close monitoring without intervention, anticoagulation, thrombolysis, thrombectomy, angioplasty, and transjugular intrahepatic portosystemic shunt. According to the EASL clinical guidelines for vascular diseases of the liver, the use of direct-acting anticoagulants is recommended. Rivaroxaban, as well as other direct oral anticoagulants, are promising drugs for the treatment of PVT due to their advantages.
