Влияние неоадъювантной химиолучевой терапии на латеральные тазовые лимфатические узлы у больных раком прямой кишки

В исследовании изучалось влияние неоадъювантной химиолучевой терапии (нХЛТ) на частоту поражения латеральных лимфоузлов (ЛЛ) при раке прямой кишки. Проанализированы результаты лечения 120 больных с диагнозом рака прямой кишки, которым выполнялась классическая тотальная мезоректумэктомия (ТМЭ) в сочетании с уни- или билатеральной тазовой лимфодиссекцией (ЛТЛД). 35 из них прошли нХЛТ, 85 пациентов – нет. С целью получения однородных сравниваемых групп больных выполнена выборка по принципу «случай-контроль». В результате образовались две группы по 32 пациента, которым выполнялась ТМЭ+ЛТЛД (контрольная группа) и нХЛТ+ТМЭ+ЛТЛД (группа сравнения). Не было получено статистически достоверной разницы в частоте поражения ЛЛ с и без нХЛТ (25 % (8/32) и 28,1 % (9/32) соответственно, р=0,08). Также не было разницы в трехлетней общей и безрецидивной выживаемости (p=0,076 и р=0,783 соответственно). Таким образом, нХЛТ не обеспечивает полную элиминацию пораженных ЛЛ, требует их удаления. нХЛТ не влияет на показатели трехлетней общей и безрецидивной выживаемости у больных раком прямой кишки. Уменьшение размера латерального лимфоузла после нХЛТ до размера менее 5 мм не свидетельствует об отсутствии метастатического поражения.
