Monocyte chemoattractant protein-1: its role in the development of tubulointerstitial fibrosis in nephropathies

Monocyte chemoattractant protein-1 (MCP-1) belongs to a large family of chemotactic cytokines that cause the migration of leukocytes in the area of inflammation. MPC-1 attracts monocytes and macrophages to the damaged area, which ultimately stimulate tubulointerstitial fibrosis.

In diabetic nephropathy, a double contour of the pathogenic effect of MPC-1 on the body was noted – the progression of nephropathy, on the one hand, and the aggravation of insulin resistance, on the other.

Increased expression of MCP-1 in renal tissue is shown with rapidly progressive and chronic glomerulonephritis, in particular, manifested by nephrotic syndrome. Binding of MCP-1 to CCR2 is accompanied by both monocyte infiltration and proliferation of epithelial, endothelial and vascular smooth muscle cells. The level of MCP-1 in the urine is also increased during the exacerbation of glomerulonephritis.

Thus, the study of MCP-1, as an important element of the response of an organism to damage of the renal structures, is relevant. Chemotactic activity of MPC-1 allows to involve monocytes, macrophages and dendritic cells in the renal zone of damage, realizing the full spectrum of elements of the immunological response. One of the most important elements in glomerular lesions is the development of tubulointerstitial fibrosis, a potent risk factor for terminal renal failure. Blockade of renal effects MCP-1 may have a positive effect on the rate of progression of renal failure in glomerulopathies of different genesis.
