The article covers the main principles and approaches that was formed in the international declaration of commitment to treatment. Along with the publication of the declaration text, its main provisions are commented on. The importance of assessing the potential adherence to treatment and a comprehensive assessment of adherence is highlighted. It is proposed to consider the adherence to treatment as a poorly corrected or non-corrected factor that decreases over time. Based on this, it is recommended in medical practice to get away from modifying the patient’s lack of commitment to modifying medical interventions based on commitment. In addition to assessing patients’ obedience to treatment, it is proposed to evaluate the devotion to treatment of their legal representatives, as well as to take into account the strict observance during the development of the preventive and medical examination programs. The need for education and training in the field of commitment of both medical personnel and medical teachers is essential.
