Duplication of the rectum is a rare congenital malformation, one of the types of duplication cysts of the gastrointestinal tract. This malformation should be remembered if patients with a fistulous form of chronic paraproctitis resist standard surgical and conservative treatment. The article presents a clinical case of a 9 year-old patient with chronic paraproctitis and recurrent pararectal fistula. Examination revealed the presence of two pararectal fistulas. According to an MRI of the small pelvis, cystic formation in the retrorectal space was revealed. The child underwent incision and drainage of pelviorectal paraproctitis four times. During hospitalization in the «cold» period, cystic masses and pararectal fistulas were excised with a posterior approach excision. Histological examination revealed a doubling of the rectum. Follow-up for two years: no complications and relapses were noted.
