Efficacy and safety of mechanical thrombectomy in combination with thrombolytic therapy in patients with ischemic stroke: a systematic review

To evaluate the clinical efficacy and safety of mechanical thrombectomy (MTE) in patients with acute ischemic stroke (AIS), the search of systematic reviews and meta-analyses was conducted in PubMed and the Cochrane Library databases. Selection criteria included meta-analyses studies that reported comparative efficacy and safety. Twenty publications were included. MTE + TLT vs thrombolytic therapy (TLT) alone statistically significantly improves functional outcomes. There was statistically significant greater mortality for TLT alone in compare with MTE + TLT. TLT showing a higher incidence of intracranial hemorrhages compared to the group of patients who use MTE + TLT. MTE + TLT has better efficacy and safety compared with TLT or MTE alone and better safety compared with TLT alone in patients with acute ischemic stroke.
