The results of treatment of 955 patients with acute adhesive obturation small bowel obstruction were analyzed. From special methods of investigation, we used radiography of abdominal cavity and abdominal ultrasound to diagnose and predict the course of intestinal obstruction. These studies were performed twice – at admission and after active decompression therapy, 6 hours later. According to the dynamics of clinical, radiographic and ultrasound data, we diagnosed a stage of disease. On the basis of this classification we designed a medical-diagnostic program that will predict the course of obstruction, determine the time of preoperative preparation, the indications for conservative and surgical treatment. It has reduced the number of operated patients to 15.94 %, thus avoiding unnecessary operations and reduced mortality to 1.37 %.
