Clinical and functional efficiency of large joint arthroplasty in patients with hignly active rheumatoid arthritis

This article presents the results of hip and knee joint replacement in patients with rheumatoid arthritis (RA) high activity and its impact on patient’s quality of life during the first year after surgery. Arthroplasty was performed in 24 patients (mean age – 56,5±9,6 years) with high activity of RA. Disease modifying antirheumatic drugs (DMARD) continued to take at the time of surgery 19 (79,1%) patients, corticosteroids (prednisone) – 10 (41.6%) patients. Joint pain (VAS), disease activity – DAS28, functional ability of the index HAQ were
assessed before the operation, after 6 and 12 months.
The intensity of joint pain (VAS) was observed in the first month after the surgery, at 6 months decreased by 1.5 times up to 48,0±10 9 mm (p<0.05,) after 12 months – up to 30,1±10,0 mm (p<0.05). Disease activity ( DAS28) reduced significantly (p<0.05) during the first year after surgery. The positive dynamics of the HAQ index was shown: after 6 months – 1,22±0,28 (p<0.05 ), at 1 year – 1,03±0,18 points (p<0.05).
Arthroplasty in patients with highly active RA may be considered quite reasonable and effective in improving functional ability, relief of pain and inflammation.
